Leading for Systems Change Terms & Conditions
Registrants for the Leading for Systems Change (LSC) require the support of the registrant’s line manager. The line manager will act as a representative of the employing organisation.
The registrant confirms to have read, and agree to, the terms and conditions of this document.
The registrant confirms that:
1.1 I have full support in place from my line manager for me to commit to all aspects of learning, and they have agreed, on behalf of my organisation, to make the necessary work place arrangements and allow me time away from work in order for me to attend the workshops, for the full duration of LSC. Please note that confirmation of your registration will be sent to your line manager.
1.2 I understand that if I move jobs during the LSC:
a) This does not automatically constitute extenuating circumstances for withdrawal if I move jobs within my Integrated Care System (ICS); and
b) I will make my new employer and line manager aware, before I accept the job offer, that I am committed to completing LSC and will require their support to do so.
c) This does constitute extenuating circumstances for withdrawal from LSC if I move jobs outside my ICS
d) I will inform the NHS Leadership Academy (the Academy) by email of the change.
1.3 I confirm that all the information provided in the registration form is up to date and accurate. If any information changes, I will update the Academy by email.
1.4 If I am from outside the United Kingdom and working in England with a visa; I confirm my UK visa and immigration clearance covers study undertaken as part of my formal employment
1.5 I will attend and partake in all elements of the LSC, regardless of the amount of study leave I am granted and understand that not attending or partaking may not provide me with the best possible learning and experience the LSC aims to offer. Non-attendance, without agreeing extenuating circumstances with the Academy could mean that I may lose my place.
1.6 I understand that LSC is designed to include practical learning in the workplace and will involve working with colleagues across the ICS on a project which is aligned to the ICS strategic priorities. I will share my learning with colleagues, acting as a ‘change agent’ and advocate for system leadership thinking and behaviours.
1.7 I understand I am applying for LSC during its early adopter phase and I am happy to provide feedback and participate in evaluation when requested to do so, which may be after I finish the LSC.
1.8 The delivery will initially take place online but should this change, I understand that my travel cost and subsistence (other than lunches during the face to face days, where applicable) are not covered and my employing organisation’s expenses policy will apply.
1.9 I understand any information I provide in completing this application will be stored and processed in accordance with the Academy’s Data Privacy Policy, which I am required to read and accept before completing my application: https://www.leadershipacademy.nhs.uk/privacy/.
1.10 I understand my anonymised information will be used for reporting purposes as part of the Academy’s work.
1.11 I understand my employing organisation will be advised of my status during the process.
1.12 I understand there may be audio recording, filming and photography of me taken whilst on LSC. I agree that the Academy and my ICS may use photographs, film, audio and video recordings of me made during LSC to showcase LSC including promoting it in the NHS, to Central Government, and to the public via the Academy’s website or other media.
1.13 I explicitly consent to the data and information sharing as outlined in sections 1.9–1.12.
1.14 I understand that I can opt out of any aspect of the data and information sharing as outlined in sections 1.9–1.12 at any time by writing to
[email protected] that I wish this information to be withheld.
1.15 I understand that, if I opt out of any data and information sharing as outlined in section 1.9–1.12, I may not be able to be offered the optimal development opportunities LSC aims to provide, and may not be able to join LSC.
1.16 I am aware that the workshops will run virtually using Microsoft Teams and the Academy will provide a technical checklist which I will complete before the cohort starts. I will make the Academy aware of any accessibility issues I have in this regard, so that appropriate support can be found. Access will also be provided to a digital ‘community of practice’ where I will be able to interact with other participants and share learning and ideas.
